Practice-Address Authorization
Conditional practice-address authorization, separately reviewed and approved.

Additional member room time
Continue booking after included hours are used, subject to room availability.
Additional member room time is billed per whole hour at room-specific rates: Consultation Room / Mental Health Studio $22/hour, Brief Visit Room $18/hour, Exam Room $22/hour, Procedure & Treatment Room $32/hour.

Membership terms at a glance
Membership Hours may be used in any active Housa room at no additional room charge, except the Procedure & Treatment Room, where each included Membership Hour used adds a $10 upgrade fee.
- Month-to-month. There is no initial commitment period.
- Membership is billed on your monthly anniversary date and is not prorated.
- Cancellation requires 7 days' notice before the next anniversary date.
- Plan changes take effect on the next anniversary date.
- Unused included hours expire each period. There is no rollover, transfer, or refund.
- Additional member room time is billed per whole hour at room-specific rates: Consultation Room / Mental Health Studio $22/hour, Brief Visit Room $18/hour, Exam Room $22/hour, Procedure & Treatment Room $32/hour.
- A half-day or full-day booking counts as one active future reservation, and paid add-on reservations count toward the same limit.
Conditional practice access
Practice-Address Authorization
- Practice-Address Authorization is available only when separately reviewed and approved by Housa. It is not automatically included with Facility Access Membership or On-Demand Facility Access.
- Eligibility is reviewed individually and practice-location authorization requires Housa's written approval.
- Authorization is conditional. Housa approval does not guarantee acceptance by licensing boards, payers, controlled-substance authorities, pharmacies, banks, registries, platforms, insurers, or other third parties.

Secure Mail Pickup is optional and remains separate from general provider approval.
